Teens are allowed. They even have spa specials for the teen clubs, however if you are under 16 you might need a parent or guardian to go with you. Check on carnival.com to read their rules.
Yes, the ships all have lots of shops and they charge it all to your sail & sign card. I don’t know about taxes (the website might mention it though). Contact the cruiseline if you really need to know and can’t find it on their website.
I’m not sure about the piano playing. You should ask though, it won’t hurt you.

Some tips..
Read the itinerary (they give you one every day, called carnival capers) and do research, so your cruise won’t go to waste. That way you’ll know all the activities going on and won’t miss out on anything you wanted to do.
If you are a teen that is travelling without other kids your age, I suggest you go to the teen clubs (Circle C for 12-14 year olds, Club 02 for 15-17 year olds) to meet other teens, if you want someone to hang out with. It’s best to go on the first night, as they get closer as the cruise progresses, and people joining near the end probably feel left out. I went and had an amazing time. The club also has lots of activities and games. It’s free, by the way.
Don’t waste all your time in the pools or hot tubs. You probably have pools at home too, and it’s a waste of money to spend your entire time in one on the cruise.
They will charge you for beverages. Some are free, however sodas, other beverages, and alcoholic drinks are really expensive. If you plan on drinking a lot of soda, get a Fountain Fun card. That way, you get unlimited soda and won’t be charged for each one. The price of the card depends on the length of your cruise.
Sign up for your shore excursions (like scuba diving) before you board the ship. You can still do it on board, but it’s a first come first serve basis and they might be full, so if you really want to go, sign up on carnival.com so that you’re guarateed to go. Waiting until the day of the excursion is a bad idea, as they might not have anymore room for you.
If you plan on going to the top of the ship at night, it gets really windy.
